Force unique value of IPC activity

There have been challenges with being able to create an IPO activity with the same number. This has been fixed so that you cannot create one - or rename one - that already exists.

Unfortunately, this original solution didn't appear in the changelog in the first release of 01.35, even though it was implemented.
